About the RoleWe are seeking a compassionate and experienced Case Manager to provide high-quality case management services to beneficiaries. In this role, you will screen, assess, and develop individualized care plans while ensuring compliance with state-specific licensure requirements and organizational policies. Your expertise will help beneficiaries navigate their healthcare journey, access essential resources, and achieve positive health outcomes.
Key Responsibilities
Conduct screenings and assessments to determine eligibility for case management services.
Develop and implement personalized care plans based on individual needs, goals, and outcomes.
Monitor and adjust care plans to ensure continued effectiveness.
Advocate for beneficiaries by ensuring access to necessary healthcare services and resources.
Provide guidance on available community programs and support networks.
Process and review authorization and referral requests while maintaining compliance with policies.
Maintain accurate documentation within the medical management system.
Collaborate with healthcare providers and internal teams to ensure alignment of care plans.
Offer oversight and guidance to non-clinical staff to support operational objectives.
Demonstrate professionalism, reliability, and clear communication in all interactions.
Required Qualifications
Current, unrestricted state license as a Licensed Clinical Social Worker (LCSW).
Minimum of two years of direct clinical care experience.
Ability to obtain a URAC-recognized case management certification within four years of hire.
U.S. citizenship and ability to pass a Department of Defense (DOD) background investigation.
Preferred Qualifications
At least two years of case management experience.
Possession of a URAC-recognized case manager certification.
Skills & Competencies
Technical Expertise: Strong knowledge of case management principles, HIPAA regulations, URAC standards, and medical coding.
Problem-Solving: Ability to analyze complex situations and develop effective care strategies.
Communication: Excellent verbal and written communication skills, adaptable to diverse audiences.
Organizational Skills: Ability to manage multiple priorities in a fast-paced environment while maintaining attention to detail.
Collaboration: Strong teamwork skills, with the ability to inspire collaboration and improve patient outcomes.
